FOR NEW MEMBERSHIP OR RENEWAL OF YOUR MEMBERSHIP

1.  Read Medical Guidelines 

2.  Download Medical Questionnaire, complete - save a copy for yourself for future reference

3.  Email form back to head coach/safety officer on the club email address.
